The Admissions Manager is responsible for the patient assessment and administrative processes and admit patients to By the Bay Health service. This includes information interviews with patients and families, review of eligibility for the Hospice benefit, physician orders, patient assessment, processing of regulatory and organizational documents. The manager is responsibility for achieving organizational metrics. The position is expected to build and maintain optimal relationships with all referral sources in compliance with organization policies and procedures and applicable laws and regulations
Schedule:
Monday - Friday; 8:30am - 5:00pm; AOC duties required
Hybrid - 3 days required onsite

Essential Duties & Responsibilities include (but are not limited to):
Provide direct supervision to both clinical and administrative staff to support the admission processes
Provide informational and admission services to patients as needed
Implement and monitor compliance with organization policies and procedures related to clinical services
Oversee activity and documentation related to patient’s initial plan of care to ensure compliance with organization procedures
Oversee supervision of admission personnel, completing written competencies, performance evaluations and skills checklist in accordance with organization policy, accreditation, and regulatory standards
Education/Experience:
Minimum of two years supervisory experience preferred, within the past seven years.
Five years experience as a professional nurse from an accredited School of Nursing
Certificates, Licenses, Registrations:
Current license as a registered nurse in the State of California
Pay Range:
$130K - $155K annually
